I understand that these messages may be sent to the mobile phone number I provide to Human Resources or any updated number I subsequently provide. Standard message and data rates may apply, depending on my mobile carrier and plan.

I further acknowledge that I may opt out of receiving SMS text messages at any time by submitting a written request to the Human Resources Department. Upon receipt of such request, JAT of Fort Wayne will take reasonable steps to remove my number from future automated SMS communications, except where such messages are required by law or essential to employment-related safety or compliance.
